Privacy Policy

When you submit a contact or appointment request form, we collect: your name, email address, phone number (optional), preferred clinic location, and a brief message. We do not collect date of birth, insurance information, medical history, or any other Protected Health Information (PHI).

This site does not set cookies. We use Plausible Analytics — a privacy-first, cookieless analytics platform that collects no personal data and is fully compliant with CCPA, GDPR, and HIPAA guidance for marketing websites.

Form submissions are forwarded via Resend (encrypted in transit over TLS) to the requested clinic's staff email. No form data is stored in a database by this website.

Analytics data collected by Plausible is aggregated and anonymous. It is used solely to understand which pages are visited and to improve the site. No individual user is tracked.

This is a public marketing website. It is not a HIPAA-covered entity in itself and does not handle medical records, prescriptions, insurance claims, or any other PHI. Patient records and clinical intake must be managed through our secure, HIPAA-certified patient portal (RevolutionEHR).

Google Maps — linked externally for directions. Clicking a map link is subject to Google's privacy policy.

Under the California Consumer Privacy Act (CCPA), California residents have the right to know what personal information is collected, to request deletion, and to opt out of the sale of personal information. We do not sell personal information.
